【摘要】:Background:Effectivehealthcaredeliveryrequiresanefficientinformationbase.AstandarddescriptionisneededregardingthecapabilitiesofRegistration-Admission,Discharge,Transfer(R-ADT)Systemsinbothautomatedhospitalandambulatorycareinformationsystems.Thispracticeisintendednotonlytoprovideacommonexplanationoftheminimuminformationelementsrequiredinsuchsystems,thusaugmentingthosealreadypublished,butalsotoprovidethebasisforfuturepatientdatainterchangeformats.ThispracticehasbeendevelopedtoserveasauniformminimumdescriptionofR-ADTfunctionalcomponentsthatshouldbecommoninallsystemsandusedinbothtransportablegeneralpurposeandcustomdevelopedsystems.Thisdescriptionrequiresacceptanceofthepremiseregardingtheneedforlogicalintegrationofconceptsinsystemsdevelopment.Intheintegratedsystemsconcept,theR-ADTfunctionisthefoundationmoduleforallpatientinformationandcommunicationamongalldepartments,anditisusedininitiatingserviceswithinthepatientcaresetting.AcommonR-ADTsysteminahospitalenablesalldepartmentstostreamlinetheinitiationandtrackingoftheservicestheyprovidetopatients;italsoprovidesanopportunityforaccuratetrackingofpatientmovementthroughoutahospitalstay,forinstance,andthelinkageofinpatientandoutpatientservices.Itisalsothesystemwhichprovidesallinpatientcensus-relatedadministrativereports.Likewise,anR-ADTcomponentinanEnterpriseArchitecturecapturestheinitialpatientdemographicprofilefortheEHRandissubsequentlyaccessedinpostinganindividual''sclinicaldata,forinquiryregardingthatclinicaldataandforlinkagetofinancialrecords.ItisanintegralpartoftheEHRfunction.Itmayalsobelinkedtoothersystemswhichprovidepatientcareinformationmanagementcapabilities.Aregistrationsystemiscapableofprovidingtheinitialinformationcaptureforallhealthcarefacilities;anADTsubsystemcanprovidecommonadmittingdataforalldepartmentsinhospitalsandotherinpatientfacilities.EstablishingastandarddescriptionofalogicalR-ADTprocessmodelisusefulbecausethatstandardwillbecomeareferenceforotherdocumentsdescribingtheotherfunctionalsubsystemsusedinpatientcareinformationsystems.Itisunderstoodthataminimumsetofinformationelementsmustbeinitiallycaptureduponregistrationandthenusedforallsubsequentambulatoryorinpatientcare;thesubsequentminimumsetofadmittingelementsisthenusedtodriveorinitiateadditionalservicesforpatientsthrougheachsubsystem.WithastandardminimumR-ADTcomponentdefinition,standardsforconstituentsubsystemscannowbecoordinatedanddevelopedthroughreferencetothismodel.Thisdescriptionshouldbeusedbyvendorsandsubsystemdesignerswhoneedtodeveloptheirsystemsinacoordinatedandintegratedwaysothateachsubsystemwillcontributemodularlywithoverallsystemsplanningfortheuserorganization.Suchmodularitywillaidmanagementwhoareassignedtoevaluateeachsystemandsubsysteminordertoassessthepotentialofexistingtechnologytoprovidetheneededpatientcareinformationmanagementsystemscapabilities.Use8212;ThispracticeiswrittenassumingthatthehealthcarefacilitywillhaveseveraloptionsforgainingtheR-ADTcapabilityandmayeitheracquireasystemfromacommercialvendorordesignanintegratedin-housesystemwhichmaybeacomponentofanambulatorycarepracticeorahospitalinformationsystem.Manyofthecharacteristicsofexistingvendorsystemsareconventionalandcaninteroperate;thecarefacilitymaysimplyneedtoidentifywhetherornottheofferedfeaturesmeetitsneeds.Beyondthegeneralcapabili.......
